Previous nasal trauma and a failed manipulation. He wanted to correct the deviation of his nose to create a straight nose from the front he wanted to remove the hook and also the dependency of his nasal tip when he smiled. The cone beam CT scan an important means of assessing the internal structures of the nose clearly shows the bony asymmetry, the septum deviation and also the restricted airway.
His surgery was with the open approach using the central platform preservation, the nasal pyramid was moved with Piezo ultrasonic technology and he had a low strip septum repositioning to stretch the convexity of his bridge to one that became straight. An additional spreader graft was used to support the concavity of the right upper lateral cartilage. The nasal tip was resuspended over the new septal position.
